We are committed to accommodating your request, however, there are instances where we may not be able to provide replacement units, as outlined below:

• Rapid exchange is only valid for TV products purchased from LG Fulfillment.
• Damage to the box matching the damaged unit was not reported at delivery.
• The damaged units do not have a matching box.
• The unit has been altered or damage was not reported within four months of receiving the unit.
• The unit has been installed.
• The unit was received at a facility and moved to another location prior to reporting damage.

1. Photos (Accepted file types: jpg, pdf, png, Max. file size: 50 MB.)
   • Serial Number Tag from Back of Unit
   • Entire Front of Box (front of box says “front in bold black line)
   • Model/Serial Number Tag on Side of Box (please make sure model and serial number are visible)
2. Order Number (ORD00######; if order number is not available, please provide PO number)
3. Contact information for the two individuals who are filing the claim.
